OUTPATIENT ULTRASOUND CLINIC FOR DIAGNOSIS OF GYNECOLOGICAL MALIGNANCIES AND SURVEILLANCE OF PATIENTS IN FOLLOW-UP FROM GYNECOLOGICAL CANCER.
In the field of gynecologic oncology, ultrasonography has assumed an increasingly significant role in recent decades. Because of its high diagnostic accuracy in the study of the female pelvis, its speed of performance and the relative low cost of the examination, ultrasound is now theexamination of first choice for the management of the patient with gynecologic malignancy.
Diagnostic findings from ultrasound examination influence clinical decisions substantially. Here are some examples:
- In the setting of ovarian masses, ultrasonography, performed by experienced operators, can discriminate, a malignant neoformation from a benign one with more than 90% accuracy. This enables appropriate planning of surgical management of patients by offering: (i) maximum surgical effort to patients with advanced ovarian carcinoma (optimal surgical cytoreduction is the main prognostic factor in these patients and this is possible only in highly specialized centers), (ii) maximum fertility preservation efforts in young women with suspected borderline tumor diagnosis or invasive cancer at an early stage who, if not carefully selected, may face inappropriate radical surgery or underestimation of their neoplasm, iii) scheduling minimally invasive surgery or scheduling longitudinal surveillance for all neoformations with ultrasound features of benignity.
- For endometrial carcinoma, which is the most frequent gynecologic neoplasm, ultrasonography has been shown to provide accurate preoperative assessment of the extent of disease, allowing optimal planning of appropriate surgical treatment.
- Also for the evaluation of cervical carcinoma, recent scientific experiences have shown that, in the assessment of the local extent of disease, transvaginal ultrasound can provide high diagnostic accuracy, allowing rapid therapeutic framing.
- In the follow-up of patients operated on for gynecologic malignancies, transvaginal pelvic ultrasound has proven to be a useful and accurate tool for early detection of pelvic recurrence.
